Sick Notes, Fit Notes and Self-Certification
You may need a sick or ‘fit’ note if you have been off work for 7 days or more.
To request a sick note, you can:
- Use the NHS App
- Use eConsult
- Contact or visit the surgery. We are open 8:00 am to 6:30 pm .
If you’ve been off for 7 days or fewer
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill.
Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
Hospital Requests
If you have had treatment or procedures performed by another Healthcare professional you should ask they issue the fit note. The GP may be unable to process requests as they have not been providing the treatment.
‘May be Fit to work’
On assessment the GP may find you fit for some work, in which case they would issue a ‘may be fit for work’ note specifying details of what/what not you are capable of.
